Every section in the files is listed as a Column in the table. As some sections can be quite large, the width of the columns is fixed, which can truncate some content. If you click on the section, it will 'pop' out, so you can read the whole section. The Summary table lets you compare a section over several documents. For example, medications prescribed over a time period. Your application will determine the filename of the CDA document to display (eg via file dialog). The filename is then passed to the webbrowser component and CDA will be rendered and displayted in the web browser. http://<server_ip>:7750/?cda=<cda_filename> cda_filename = Full path, name and xml extension of the CDA document. server_ip = IP address or domain name of the server. Where the CDA Viewer and the browser are on the same PC, the server_ip is 127.0.0.1. Where the server is on a network, the IP address of the server can be used. Delphi Example Drop a TwebBrowser component on a windows form. Pass the filename string webBrowser1.Navigate("http://127.0.0.1:7750/?cda=C:\\Intelsoft\\CDA%20Viewer\\Medical%20Documents\\demo%20documents\\C-CDA_R2-1_CCD.xml"); unit Unit3; { Sample Delphi client what will display a C-CDA document. Requires the CDAViewerSvc to be running. Drop a TButton, TWebBrowser and TOpendialog on a form. The user can click on 'Open CDA' to select a CDA document, which will be displayed in the TWebBrowser. The WebBrowser just needs to navigate to : http://<server_ip>:7750/?cda=<cda_filename> } interface uses Winapi.Windows, Winapi.Messages, System.SysUtils, System.Variants, System.Classes, Vcl.Graphics, Vcl.Controls, Vcl.Forms, Vcl.Dialogs, Vcl.StdCtrls, Vcl.OleCtrls, SHDocVw, Vcl.ExtCtrls; type TForm3 = class(TForm) OpenDialog1: TOpenDialog; Panel1: TPanel; Button1: TButton; WebBrowser1: TWebBrowser; procedure Button1Click(Sender: TObject); private { Private declarations } public { Public declarations } end; var Form3: TForm3; implementation {$R *.dfm} procedure TForm3.Button1Click(Sender: TObject); var s: string; begin if opendialog1.Execute then begin s:='http://localhost:7750/?cda='+opendialog1.FileName; WebBrowser1.Navigate(s); end; end; end. All of these documents are displayed in the Viewer in the same way, aside from the fact that they contain different content. A CDA document is prepared by a doctor to record a medical episode, such as a hospital admission or a visit to a clinic. Each CDA document is about a single patient and is a part of the medical record of that patient. A CDA document can contain a lot of detail. It is designed to be read by people and to be used to transfer information between the IT systems of different health care providers. A CDA document will contain several sections. Each section describes a small part of the medical record, such as 'Reason for visit', 'Results', 'Procedures', and so on. The CDA Viewer displays these sections so that they are easy to understand. You are able to move the sections around, hide/show and collapse/expand these sections as you wish, depending on which part of the CDA document you want to read. Opening C-CDA Documents The Medical Record Viewer file toolbar is used to open CDA documents, and is shown below: Open File - Click this to open a single CDA file. CDA files have a '.xml' or '.XML' extension. Open Folder - Click this to open a folder containing a set of CDA documents. The Viewer will display: any sub folders in the folder selected, allowing you to easily navigate between different folders. You can click on any sub folder to see the CDA files in the folder. a list of all CDA files in the folder You can click on any CDA file in the displayed list to View it. Save File - click this to save any changes you have made to a CDA file. Reload File - click to reload the CDA file in the Viewer. Close File - click this to close the current CDA file and remove it from the file list. Navigation: Ordering, Showing and Displaying Sections You can re-order and hide sections as you wish. Note that your preferences for section display are saved as you change them. The preferences are applied when you open any subsequent CDA document. For example, if you move the 'Procedures' section to the top and then hide the 'Results' section of a CDA document, these preferences are saved. That is, when you open a new document, the 'Procedures' seciton will be at the top and the 'Results' section will be hidden. Ordering Sections There are a few ways you can re-order the sections in the CDA document you have opened. Drag and drop a section: click and hold on a section, then drag it to where you want it. Click on move to top: The move to top button looks like this: There is a move to top button in the title bar of each section and next to each section in the Table Of Contents. If you click on the move to top button, the section will be placed at the start of all sections. Click on move down one: The move down one button looks like this: There is a move down one button in the title bar of each section and next to each section in the Table Of Contents. If you click on the move to top button, the section will swap places with the section below it. Showing and Displaying Sections When you hover over a Section, the section toolbar is displayed: The easiest way to see what each of these buttons does is to simply click on it. Collapse / Expand: Expands or collapses the section. Use this if you want the section to ramin in the CDA, but don't want to see all of the text in the section. Move to top: Click on this to move the section to the top of the CDA document. Move down one: Click on this to make the section swap position with the section below it. Hide: Click on this to hide the section from the CDA document. This will not alter the file itself, but it will hide the section from display. You can show (unhide) the section from the Table of Contents. Feedback/Annotate: Click on this to add your own content to the CDA document. Changes you make will be saved in a new version of the CDA, leaving the original unchanged. Notified/Reviewed: Click on this to place a check mark on the section, which can indicate that you have reviewed the section. Table Of Contents The Table Of Contents (TOC) is on the right hand side of the Viewer. You can collapse / expand the TOC by clicking on its title. The TOC shows the current order of all sections in the CDA Document, including hidden sections. You can show / hide a section by selecting the checkbox next to the section title. You can move each section by clicking on the up and down buttons next to a section. Under the TOC are additional buttons: Resore original order: This button will restore the order of the sections to that of the source CDA Document. Show all: This button will show all sections, that is, it makes all hidden sections visible again. Collapse/Expand all: This button will collapse or expand all of the sections at once. brynlewis@backbeachsoftware.com.au (Administrator)Tue, 17 May 2022 18:08:09 +1000WebQI for Neuromodulation deployedhttp://backbeachsoftware.com.au/?cmd=getsql&id_site=1&id_topic=68http://backbeachsoftware.com.au/?cmd=getsql&id_site=1&id_topic=68A Neuromodulation project has gone live at Epworth Health. This deployment is the first to implement the PMHC MDS (Primary Mental Health Care - Minimum Dataset). Further deployments for PMHC MDS are imminent, as it is a major area in which PHNs are commissioning service providers. WebQI has been selected to underpin the data collection for commissioning activities for SEMPHN and as such, other projects are being implemented at present. The figure below shows the PMHC MDS, with an extension to accomodate MHNiP sessions and invoices. The data model is taken from https://docs.pmhc-mds.com/data-specification/data-model-and-specifications.html#data-model. The extensions are shown in blue. A session is created by aggregating Service_Contacts into a Session. This application has been developed to extend the functionality of the online C-CDA Viewer. Medical Record Viewer - Help Manual Personal privacy and security: The Viewer is a standalone application that allows you to view personal medical records on a desktop without uploading files to a server. Multiple document loading: You can load all the files in a directory, or a single file at a time. Each file is loaded in a tab and retained in the Viewer until closed, allowing you to switch between files. Structure and Validation reporting of a batch or single document: As files are loaded, they are checked for structural or validation errors. On loading a directory, a report detailing each file is presented, including error messages. Patient feedback collection: You can add feedback to a C-CDA document. The feedback may be a note which is added to the C-CDA structure, retaining date and authorship information. The original file is not altered - a new document is created when notes are added. The new document is the next version, with links to the original document. Links between versions of a document are indicated by color coding. Changes are timestamped and patient authorship is added. Review functionality: A review process can be recorded within the document, recording who has reviewed the sections in a document. Quality assurance checking: The machine readable version of the document can be viewed and easily comparison to the narrative text. You can view the XML of a section or entry within the Viewer. The XML hierarchy and certain of the attributes can be hidden/shown, allowing the data of the record to be examined and compared to the narrative text. Document details including author are immediately accessible. Provenance and authorship assessment: Where a section or entry author differs to the document author, the additional author is listed, highlighting these differences to readers. Visual indicators of Section linking: Links between document sections are indicated with an icon. Clicking the icon will highlight the linked section. For example, a Health concern may link to a Goal. Access Logging: All access to files is logged, recording file, document detail, user and date of access. System Requirements: Download and run the installer. The Viewer is a Windows application, tested on Windows 7 and Windows 10. Bug fixes: Ordering sections on drag and drop will now update the TOC as it is occurring. Previously this was only updated after a reload. Hidden sections warning icon is now updated correctly in real time. Previously, the warning icon was not removed after that last hidden section was made visible. Original Features: Easy-to-use viewer of complex C-CDA documents, available directly in any web browser. A responsive document layout automatically adjusts to make optimum use of the available screen space. Users can hide, collapse and move any section of a CDA document. Document sections can be manipulated directly via button clicks, drag and drop or via a document Table Of Contents All user preferences are saved and automatically applied to subsequent CDA documents opened Ability to detect and hide/show duplicates The result is an intuitive and easy to use document layout that users can control directly. All this using simple deployment via a local fileset or web server (and it's very pretty). brynlewis@backbeachsoftware.com.au (Administrator)Tue, 17 May 2022 18:08:09 +1000Downloadhttp://backbeachsoftware.com.au/?cmd=getsql&id_site=1&id_topic=60http://backbeachsoftware.com.au/?cmd=getsql&id_site=1&id_topic=60Download the Backbeach Software Medical Record Viewer installer here. The Oncology and Stomal Care projects, along with the existing Breast Care project, provide a patient record for specialist nursing. The nurse records a brief patient profile and then records each of the activities they carry out for the patient during a patient interaction, as well as the time taken for the activity. The data collected can then be used to provide comprehensive reports on activities, using WebQI reporting functionality. These projects were the first to use the project editor in WebQI. The project editor allows end users to specify the wording, structure and layout of the questions in the data collection of the project. Users can manipulate the data collection form in a WYSIWYG editor, with access to full editing capabilities. The editor speeds up the development process, as users can create the forms themselves, with little or no input needed from developers. Project managers can consult directly with their user group and create the draft form immediately. brynlewis@backbeachsoftware.com.au (Administrator)WebQIClientsEpworth HealthCareTue, 17 May 2022 18:08:09 +1000 ESP App - Extended Stay Patient Management Applicationhttp://backbeachsoftware.com.au/?cmd=getsql&id_site=1&id_topic=56http://backbeachsoftware.com.au/?cmd=getsql&id_site=1&id_topic=56 The ESP App aids in the management of extended stay patients and associated risk factors. Patients that stay in hospital more than 21 days are 2% of the admission episodes but 25% of the bed days, hence there is significant potential to free up resources and improve care for this class of patients. An Extended Stay refers to those patient episodes of care which are in the top 25% for LOS of the respective specialty unit. The ESP App is designed for use on an iPad or tablet, but can be used from any web browser. The App is used as a communication tool within a clinical team. It is used within a set of broader processes. Its role is to allow the team to have an informed view of all current patients and the risk of Extended Stay episodes occurring. Staff awareness is increased and hence they have increased ability to plan and take action prior to an Extended Stay occurring. The App also facilitates taking action directly and communicating status to other Hospital Departments that may have an influence on patient LOS. Patient details are sourced directly from the PAS, using a HL7 messaging connection. Location and unit are automatically updated, ensuring ward and unit lists are always accurate. As part of assessment, a patient is flagged as Low Risk High Risk Top 25% of LOS for the Unit Top 5% of LOS for the Unit Not profiled (yet) In addition, an overlay can indicate the following: Discharge is blocked Patient going home within 3 days For each patient, a set of Risk Factors can be set. Each of these is timestamped and can be reversed, allowing a dynamic risk profile for each patient to be created. The Risk Profile of a patient is determined by a dynamic rules engine. The rules take each Risk factor, apply a weighting, and assign a Risk Profile. The risk profile flag and overlay of each patient is easily seen in the patient lists. Patient lists can be created by Ward or Unit and can be ordered directly by the user. Each Risk factor has guidance associated with it, which is available to the user when the patient is assessed as High Risk. A Messaging and Notes function allows users to enter notes regarding each patient, resulting in the compilation of a history of notes. Notes can be flagged for a response by other Hospital Departments. For example, a message could be sent to Theatre, requesting urgent scheduling of a procedure. brynlewis@backbeachsoftware.com.au (Administrator)TaskboardProductsHL7 IntegrationWebQIESPTue, 17 May 2022 18:08:09 +1000Backbeach Software Wins HL7/ONC C-CDA Rendering Tool Challengehttp://backbeachsoftware.com.au/?cmd=getsql&id_site=1&id_topic=55http://backbeachsoftware.com.au/?cmd=getsql&id_site=1&id_topic=55Update - A new verion of the C-CDA Viewer has been developed and can be downloaded here Backbeach Software is very pleased to announce that it has won the HL7/ONC Tool Challenge, with the Intelsoft C-CDA Viewer. First prize is $15,000 (USD) and free registration to HL7's 30th Annual Plenary and Working Group Meeting (September 18-23, 2016) in Baltimore, Maryland where the winning tool will be demonstrated. HL7 and the Office of the National Coordinator for Health Information Technology (ONC) held the challenge to encourage the development of HL7 tools, with entries developed and submitted between January and May 2016. The winning tool renders any CDA document in a web browser, creating an easy to read and navigable view of the document. The view is responsive, can be adjusted to user preferences and will make optimal use of the available screen real estate. It is also very pretty. See a demonstration here Several further enhancements of the tool are possible. It is hoped that additional feedback will be received at the HL7 Working Group Meeting, which may find its way into future versions of the tool. The new project is the first specialist nursing project deployed on WebQI, in this case it is for Breast Care. The project will allow clinical audit data collection and reporting for specialist nurses working in Breast Care. A supporting project, Nursing Administration Activity, will allow time tracking of all nursing activity. The new projects will replace excel based tools and it is expected that this will lead to more accurate and consistent data collection. The information collected will be used to develop KPI's around response to referral, quantify specialist nurse interactions with patients, quantify and specify ongoing referrals for patients and provide workload data to support business cases. Similar projects in other specialist nursing clinical areas are planned to follow.brynlewis@backbeachsoftware.com.au (Administrator)WebQIClientsEpworth HealthCareTue, 17 May 2022 18:08:09 +1000 WebQI at Central Gippsland Health Servicehttp://backbeachsoftware.com.au/?cmd=getsql&id_site=1&id_topic=53http://backbeachsoftware.com.au/?cmd=getsql&id_site=1&id_topic=53 Central Gippsland Health Service (CGHS) has selected WebQI to provide electronic clinical audit to surgical speciality units. Access the viewer here. Additional functionality is being developed and comments or feedback is encouraged. You can accces the extended tool here Documentation 5 minute video demonstration 6 slide PDF presentation The tool is the winning entry in the C-CDA Rendering Tool Challenge. The github readme follows: Synopsis Easy to use rendering of complex C-CDA documents directly in any web browser. Responsive document layout automatically adjusts to make optimum use of the available screen space. Users can hide, collapse and move any section of a CDA document. Document sections can be manipulated directly via button clicks, drag and drop or via a document Table Of Contents (TOC). All user preferences are saved and automatically applied to subsequent CDA documents opened. Ability to detect and hide/show duplicates. The result is an intuitive and easy to use document layout that users can control directly. All this using simple deployment via a local fileset or web server (and its very pretty). Sample documents are included in the deployment. Any C-CDA document can be rendered via cut and paste of the document text. Platform Runs in any modern browser (tested on FF 46.0, IE11 and Chrome 49, Safari). PC, tablet or mobile. Will run as a standalone local fileset (not in Chrome, due to security issues) Load onto any web file server to run on Chrome Features Users can control document layout via Section hide/show, collapse/expand and re-ordering. As user makes changes, the layout adjusts to make best use of available screen space. All changes are saved as preferences that will apply across documents (ie open a new document and the same order/visibility preferences are applied against all sections). Sections are identified via coding applied across documents. eg. If 'Procedures' has been moved to the top of the document, then 'Procedures' will be at the top of the next document opened. Sample documents are available, or you can cut and paste the xml of a C-CDA document into the text box, then click 'View'. Duplicate entries in tables are detected. The user can decide whether these are hidden or shown. Move and Re-Ordering Drag and Drop a section where you want it to go. Move a section up to the top of the list by clicking on the double-up arrow. Move a section down one by clicking on the single-down arrow. Show/Hide Sections Hide sections by clicking on the cross, or from the Table Of Contents (TOC) Collapse/Expand sections Click the collapse/expand icon on any section. Collapse/Expand all section with the Collapse/Expand all button. TOC (Table Of Contents) Use the TOC to see all available sections in the document, and to show/hide or move sections. Reset all preferences to defaults. Deployment Local Deployment Download the fileset. Open 'index.htm' in a web browser. Web Deployment Download the fileset and save to a public directory (eg. C-CDA_Viewer). The viewer is then available at: http://yoururl/[C-CDA_Viewer_path]/index.htm Enjoy. Implementation core.js: C-CDA specific processing is carried out in core.js. xslt.js: a javascript library that performs xsl transformations in the browser. You access Data Analytics from the home screen, after login. For the project you are inetrested in, click 'Show more', then 'View Data Analytics' You can select any field for inclusion as a column in the grid. The screenshot shows that Major procedure and Additional procedure have been selected. If you select a field that has a menu input (such as procedure or diagnosis), the grid will contain additional columns (All) column name. This column will contain the values of all fields associated with the menu column name. This column will contain the values of the field concerned The screenshot demonstrates this: If you select several fields associated with the menu, you will get an '(All) column name' column for each of them. This column will be a duplicate. Bug fixes The above functionality now reflects the column names appropriately. Selection of multiple menu-associated fields (eg diagnosis and procedure) will layout and display column names correctly. Fixed incorrect display of deleted records in the table listing under each node. brynlewis@backbeachsoftware.com.au (Administrator)BlogWebQI v.1.7.19Tue, 17 May 2022 18:08:09 +1000Bloghttp://backbeachsoftware.com.au/?cmd=getsql&id_site=1&id_topic=47http://backbeachsoftware.com.au/?cmd=getsql&id_site=1&id_topic=47YABlogbrynlewis@backbeachsoftware.com.au (Administrator)Tue, 17 May 2022 18:08:09 +1000 Epworth Go-Livehttp://backbeachsoftware.com.au/?cmd=getsql&id_site=1&id_topic=45http://backbeachsoftware.com.au/?cmd=getsql&id_site=1&id_topic=45WebQI projects for General Surgery and Spinal Surgery have gone live at Epworth Health. A typical workflow would be as follows: A nurse creates a task on taskboard, eg 'review antibiotics'. Patient details are retrieved by WebQI from the patient administration system. Patient unit, cover, ward and bed are assigned by the system (based on PAS details). Background and task detail are added by the nurse. The task is added to taskboard, showing the list of tasks for a given cover doctor. Taskboard users can filter tasks by cover (eg front, back, surgical and paediatric cover), ward and unit. The assigned after hours cover doctor can acknowledge the task in a single click. Acknowledged tasks are time stamped, so the requestor knows the task has been seen. A 'reply' function allows the cover doctor to ask any clarifying questions, or supply a status update for an ongoing task Once the task is completed, the cover doctor marks the task completed. All staff can view the list of current tasks and can track the staus of individual tasks, as well the current task load. The user interface has emphasised simplicity, so as to make creating, tracking and completing tasks as straightforward as possible. Communication of current task activity is transparent, leading to all staff being more informed. As a result, paging is reduced to urgent tasks only and less time is wasted on unnecessary communications (e.g. no status checks are needed).brynlewis@backbeachsoftware.com.au (Administrator)WebQIHL7 IntegrationMonash HealthTaskboardTue, 17 May 2022 18:08:09 +1000 Taskboard Use Expandshttp://backbeachsoftware.com.au/?cmd=getsql&id_site=1&id_topic=43http://backbeachsoftware.com.au/?cmd=getsql&id_site=1&id_topic=43Taskboard is now being used for all non-urgent after hours tasks at Monash Medical Centre, Clayton. It is currently being used by the paediatrics team at Monash Childrens. Taskboard is a web based communication system that replaces paging of after hours cover doctors for non-urgent tasks. As a result, paging is reduced to urgent tasks only and less time is wasted on unnecessary communications (e.g. no status checks are needed).brynlewis@backbeachsoftware.com.au (Administrator)HL7 IntegrationMonash HealthTaskboardWebQITue, 17 May 2022 18:08:09 +1000 St Vincent's Private Hospital, Melbournehttp://backbeachsoftware.com.au/?cmd=getsql&id_site=1&id_topic=7http://backbeachsoftware.com.au/?cmd=getsql&id_site=1&id_topic=7WebQI is being used as a platform to implement the Quality In Action program at St Vincent's Private Hospital, Melbourne (SVPHM). Following the passage of legislation, from 1 December 2010 all non-exempt drugs on F2 of the Schedule of Pharmaceutical Benefits will be subject to EAPD. More information is available here. Critical success factors: Privacy and security of all data and processes Accuracy and transparency of all processes No tolerance for any timeline slippage, due to the preset dependencies in PBS pricing and scheduling. Windows App Client (Price Disclosure Submission Utility - PDSU) Each RP has a PDSU app and installer created for their use. Their App installation contains the PKI key and certificate specific to their usage. Auto-generated web form presents all drug products for the RP. The PDSU allows each RP to enter the required information into a web form, or to upload a formatted CSV file. The App encrypts all communications and submitted data. The encrypted data is then transmitted via an encrypted protocol (https). Price Disclosure Server Web Server which communicates with the PDSU of each RP, sneding and recieving PD submission data. All data is encrypted prior to transmission. The encrypted data itself is communicated via an encrypted channel (https) Has a set of import processes that allows preparation of PBS PD data for each month, which require accurate presentation of each RP's responsibilities. Carries out the Price Disclosure calculations and explanatory reporting. Has the ability to adapt to changing PD regulations. The operational requirements of this policy directed the Department to seek tenders from capable and experienced organisations to provide data collection, calculations and processing services to support the ongoing requirements of EAPD. ISS is providing the software development services required for the provision of Price Disclosure, including collecting data submissions from pharmaceutical suppliers, undertaking Price Disclosure related calculations. The software includes a secure electronic submission tool and a corresponding drug database against which all submissions are made. brynlewis@backbeachsoftware.com.au (Administrator)ClientsDepartment of HealthTue, 17 May 2022 18:08:09 +1000 Active Directory Integrationhttp://backbeachsoftware.com.au/?cmd=getsql&id_site=1&id_topic=27http://backbeachsoftware.com.au/?cmd=getsql&id_site=1&id_topic=27WebQI integrates directly with Active Directory. The PSD is a program of the Victorian Department of Education and Early Childhood Development (DEECD). The PSD provides a range of supports and initiatives to assist government schools to support students with disabilities. The objective is to help maximise student's potential growth in education and learning, and ensure that students with disabilities are valued and participate in all aspects of school life. The PSD is one of a suite of programs and interventions designed to meet the diverse educational needs of students with disabilities in Victorian government schools. The PSD administration system is a purpose built web based system, designed to implement a complex workflow that parallels the application assessment process of the PSD. The PSD system specifies a set of assessment activities that are undertaken for each application. Based on the information entered within each activity, the workflow management sub-system then directs the user to the next activity. For each application, the system indicates: the current status of each application the activity history of the application (activities undertaken, including user and date/time and activity outcome) For all applications, the system reports on the number of applications at each stage of the assessment process. The system includes a permissions model, which allows the specification of which user types can view or edit each of the activity types. This allows the separation (for example) of administration and assessmenta activities. In addition, the system manages the large volume of documentation that is associated with each application.
