All of these documents are displayed in the Viewer in the same way, aside from the fact that they contain different content. A CDA document is prepared by a doctor to record a medical episode, such as a hospital admission or a visit to a clinic. Each CDA document is about a single patient and is a part of the medical record of that patient. A CDA document can contain a lot of detail. It is designed to be read by people and to be used to transfer information between the IT systems of different health care providers. A CDA document will contain several sections. Each section describes a small part of the medical record, such as 'Reason for visit', 'Results', 'Procedures', and so on. The CDA Viewer displays these sections so that they are easy to understand. You are able to move the sections around, hide/show and collapse/expand these sections as you wish, depending on which part of the CDA document you want to read. Opening C-CDA Documents The Medical Record Viewer file toolbar is used to open CDA documents, and is shown below: Open File - Click this to open a single CDA file. CDA files have a '.xml' or '.XML' extension. Open Folder - Click this to open a folder containing a set of CDA documents. The Viewer will display: any sub folders in the folder selected, allowing you to easily navigate between different folders. You can click on any sub folder to see the CDA files in the folder. a list of all CDA files in the folder You can click on any CDA file in the displayed list to View it. Save File - click this to save any changes you have made to a CDA file. Reload File - click to reload the CDA file in the Viewer. Close File - click this to close the current CDA file and remove it from the file list. Navigation: Ordering, Showing and Displaying Sections You can re-order and hide sections as you wish. Note that your preferences for section display are saved as you change them. The preferences are applied when you open any subsequent CDA document. For example, if you move the 'Procedures' section to the top and then hide the 'Results' section of a CDA document, these preferences are saved. That is, when you open a new document, the 'Procedures' seciton will be at the top and the 'Results' section will be hidden. Ordering Sections There are a few ways you can re-order the sections in the CDA document you have opened. Drag and drop a section: click and hold on a section, then drag it to where you want it. Click on move to top: The move to top button looks like this: There is a move to top button in the title bar of each section and next to each section in the Table Of Contents. If you click on the move to top button, the section will be placed at the start of all sections. Click on move down one: The move down one button looks like this: There is a move down one button in the title bar of each section and next to each section in the Table Of Contents. If you click on the move to top button, the section will swap places with the section below it. Showing and Displaying Sections When you hover over a Section, the section toolbar is displayed: The easiest way to see what each of these buttons does is to simply click on it. Collapse / Expand: Expands or collapses the section. Use this if you want the section to ramin in the CDA, but don't want to see all of the text in the section. Move to top: Click on this to move the section to the top of the CDA document. Move down one: Click on this to make the section swap position with the section below it. Hide: Click on this to hide the section from the CDA document. This will not alter the file itself, but it will hide the section from display. You can show (unhide) the section from the Table of Contents. Feedback/Annotate: Click on this to add your own content to the CDA document. Changes you make will be saved in a new version of the CDA, leaving the original unchanged. Notified/Reviewed: Click on this to place a check mark on the section, which can indicate that you have reviewed the section. Table Of Contents The Table Of Contents (TOC) is on the right hand side of the Viewer.You can collapse / expand the TOC by clicking on its title. The TOC shows the current order of all sections in the CDA Document, including hidden sections.You can show / hide a section by selecting the checkbox next to the section title. WebQI has been selected to underpin the data collection for commissioning activities for SEMPHN and as such, other projects are being implemented at present. The figure below shows the PMHC MDS, with an extension to accomodate MHNiP sessions and invoices. Medical Record Viewer - Help Manual Personal privacy and security: The Viewer is a standalone application that allows you to view personal medical records on a desktop without uploading files to a server. Multiple document loading: You can load all the files in a directory, or a single file at a time. Each file is loaded in a tab and retained in the Viewer until closed, allowing you to switch between files. Structure and Validation reporting of a batch or single document: As files are loaded, they are checked for structural or validation errors. On loading a directory, a report detailing each file is presented, including error messages. Patient feedback collection: You can add feedback to a C-CDA document. The feedback may be a note which is added to the C-CDA structure, retaining date and authorship information. The original file is not altered - a new document is created when notes are added. The new document is the next version, with links to the original document. Links between versions of a document are indicated by color coding. Changes are timestamped and patient authorship is added. Review functionality: A review process can be recorded within the document, recording who has reviewed the sections in a document. The task list presentation is responsive to screen size. Tasks are listed in a single or double column and menus are slide in or hidden depending on what will fit on the screen. Tasks can re-ordered using drag and drop. The task list is presented in a summary or detail view, with the user able to expand each task individually, or all at once. Offline usage: Allows users to access the tasklist without a network connection. Changes made are synched to the server when the device reconnects. The data collected can then be used to provide comprehensive reports on activities, using WebQI reporting functionality. These projects were the first to use the project editor in WebQI. The project editor allows end users to specify the wording, structure and layout of the questions in the data collection of the project. Users can manipulate the data collection form in a WYSIWYG editor, with access to full editing capabilities. The editor speeds up the development process, as users can create the forms themselves, with little or no input needed from developers. Patients that stay in hospital more than 21 days are 2% of the admission episodes but 25% of the bed days, hence there is significant potential to free up resources and improve care for this class of patients. An Extended Stay refers to those patient episodes of care which are in the top 25% for LOS of the respective specialty unit. The ESP App is designed for use on an iPad or tablet, but can be used from any web browser. The App is used as a communication tool within a clinical team. It is used within a set of broader processes. Its role is to allow the team to have an informed view of all current patients and the risk of Extended Stay episodes occurring. Staff awareness is increased and hence they have increased ability to plan and take action prior to an Extended Stay occurring. The App also facilitates taking action directly and communicating status to other Hospital Departments that may have an influence on patient LOS. Patient details are sourced directly from the PAS, using a HL7 messaging connection. Location and unit are automatically updated, ensuring ward and unit lists are always accurate. As part of assessment, a patient is flagged as Low Risk High Risk Top 25% of LOS for the Unit Top 5% of LOS for the Unit Not profiled (yet) In addition, an overlay can indicate the following: Discharge is blocked Patient going home within 3 days For each patient, a set of Risk Factors can be set. Each of these is timestamped and can be reversed, allowing a dynamic risk profile for each patient to be created. The Risk Profile of a patient is determined by a dynamic rules engine. The rules take each Risk factor, apply a weighting, and assign a Risk Profile. The risk profile flag and overlay of each patient is easily seen in the patient lists. Patient lists can be created by Ward or Unit and can be ordered directly by the user. Each Risk factor has guidance associated with it, which is available to the user when the patient is assessed as High Risk. The assigned after hours cover doctor can acknowledge the task in a single click. Acknowledged tasks are time stamped, so the requestor knows the task has been seen. All staff can view the list of current tasks and can track the staus of individual tasks, as well the current task load. It is currently being used by the paediatrics team at Monash Childrens. Taskboard is a web based communication system that replaces paging of after hours cover doctors for non-urgent tasks. A typical workflow would be as follows: A nurse creates a task on taskboard, eg 'review antibiotics'. Patient details are retrieved by WebQI from the patient administration system. Patient unit, cover, ward and bed are assigned by the system (based on PAS details). Background and task detail are added by the nurse. The task is added to taskboard, showing the list of tasks for a given cover doctor. Taskboard users can filter tasks by cover (eg front, back, surgical and paediatric cover), ward and unit. The assigned after hours cover doctor can acknowledge the task in a single click. Acknowledged tasks are time stamped, so the requestor knows the task has been seen. A 'reply' function allows the cover doctor to ask any clarifying questions, or supply a status update for an ongoing task Once the task is completed, the cover doctor marks the task completed. All staff can view the list of current tasks and can track the staus of individual tasks, as well the current task load. The user interface has emphasised simplicity, so as to make creating, tracking and completing tasks as straightforward as possible. Communication of current task activity is transparent, leading to all staff being more informed. This expected LOS is in comparison to a national cohort. At suitable intervals (eg. fortnightly), the LOS of all patients is reviewed. The patients are listed in priority order, with those patients whose LOS is above a threshold listed first. Windows App Client (Price Disclosure Submission Utility - PDSU) Each RP has a PDSU app and installer created for their use. Their App installation contains the PKI key and certificate specific to their usage. Auto-generated web form presents all drug products for the RP. The PDSU allows each RP to enter the required information into a web form, or to upload a formatted CSV file. The App encrypts all communications and submitted data. The encrypted data is then transmitted via an encrypted protocol (https). Price Disclosure Server Web Server which communicates with the PDSU of each RP, sneding and recieving PD submission data. All data is encrypted prior to transmission. The encrypted data itself is communicated via an encrypted channel (https) Has a set of import processes that allows preparation of PBS PD data for each month, which require accurate presentation of each RP's responsibilities. Carries out the Price Disclosure calculations and explanatory reporting. Has the ability to adapt to changing PD regulations. The operational requirements of this policy directed the Department to seek tenders from capable and experienced organisations to provide data collection, calculations and processing services to support the ongoing requirements of EAPD. About WebQI WebQI is an online data collection, analysis and reporting application, supporting clinical quality improvement. As a large scale, time sensitive government project, the ability to reliably deliver the infrastruture needed is essential. The PSD is a program of the Victorian Department of Education and Early Childhood Development (DEECD). The PSD provides a range of supports and initiatives to assist government schools to support students with disabilities. The objective is to help maximise student's potential growth in education and learning, and ensure that students with disabilities are valued and participate in all aspects of school life. The PSD is one of a suite of programs and interventions designed to meet the diverse educational needs of students with disabilities in Victorian government schools. The PSD administration system is a purpose built web based system, designed to implement a complex workflow that parallels the application assessment process of the PSD. The PSD system specifies a set of assessment activities that are undertaken for each application. Based on the information entered within each activity, the workflow management sub-system then directs the user to the next activity. For each application, the system indicates: the current status of each application the activity history of the application (activities undertaken, including user and date/time and activity outcome) For all applications, the system reports on the number of applications at each stage of the assessment process. The system includes a permissions model, which allows the specification of which user types can view or edit each of the activity types. This allows the separation (for example) of administration and assessmenta activities. In addition, the system manages the large volume of documentation that is associated with each application.
